The difference is the service relationship
Payroll software processes pay and can include tax-filing services. A professional employer organisation (PEO) adds a co-employment arrangement and a wider division of HR responsibilities.
ADP TotalSource describes shared responsibilities: it provides benefits administration and specialist guidance while the client keeps day-to-day management and hiring decisions. The contract determines the split for the service you buy.

At 30 employees, the fee gap is large enough to demand a reason
Justworks publishes both payroll and PEO prices, which makes the difference in purchase scope visible. These calculations assume 30 billable employees for 12 unchanged months, with no promotional discount.

Plus includes medical, dental and vision administration; Basic is a different scope. The Plus service-fee gap over Payroll is $3,430 a month, or $41,160 a year. That is not a saving you can automatically claim by choosing payroll: payroll alone has not replaced the extra services.
Insurance premiums, employer taxes, wages and other charges are outside this fee comparison. Get those amounts into the total before choosing.
What would make the PEO premium worth paying?
A PEO may be worth the extra fee if it offers more suitable benefits or takes over HR work you would otherwise pay for separately. Compare the proposed benefits, support and administration with your actual alternatives.
Compare the PEO proposal with payroll plus your broker, HR support and retained administration. Include only costs the PEO would replace. If you keep the same internal HR workload, it is not a removed cost.
Do not assume health cover is cheaper because it comes through a PEO. Compare employer contributions, plan design and access—not just a premium headline. A lower bill for a materially different plan is a different decision.
Multi-state payroll is already a standalone purchase
OnPay lists multi-state payroll in its base plan at $49 plus $6 per worker monthly. For 30 workers, that is $229 a month before optional services and taxes.
This does not make OnPay equivalent to a PEO. It does show why “we now have people in two states” is not enough, by itself, to choose the broader arrangement. Separate the payroll need from the HR work the expansion creates.

A PEO does not make every employer responsibility disappear
Keep the responsibility split explicit. The IRS describes specific federal employment-tax treatment for certified PEO arrangements; it is not a blanket transfer of every employment obligation. Treatment also differs for worksite and non-worksite employees.
For the buying decision, establish which entity contracts with you and what it handles. Your managers still need to run the business, supply accurate information and own the decisions retained by the agreement.
Compare payroll plus separate HR support with a PEO
Start with payroll software if you have a capable owner for HR and benefits. Shortlist a PEO when a wider service could remove a recurring burden or offer a benefits arrangement worth changing for.

Questions buyers ask
Does hiring in two states mean we need a PEO?
No. Multi-state payroll is sold separately, including in OnPay’s published base plan. Consider a PEO for the wider HR and benefits arrangement, not the second state alone.
Are PEO health-insurance premiums included in the service fee?
Do not assume that. The Justworks fee example here compares service fees and benefits administration scope, not insurance premiums or the full employer contribution.
Is payroll software always the cheaper overall choice?
Its fee can be lower, but the complete comparison includes separate HR support, benefits administration and work retained internally. Count only services and costs the PEO actually replaces.
